You’ll have one less thing to do when you arrive for your first appointment. The last thing you want to do is fill out paperwork while juggling your insurance card, driver’s license, and other belongings. After you schedule your appointment, we will text you the patient forms you need to fill out! Complete the forms on your phone, and you’ll be ready to go when you arrive.
